Title:     The benefit of time-of-flight in PET imaging: Theoretical, experimental, and clinical results

Abstract: As the utilization of Positron Emission Tomography (PET), in combination with CT, has grown in the last several years, it has become clear that improved lesion detection and quantification are critical goals for cancer studies. These improvements are made possible by advancements in imaging technology, including development of new scintillation detectors, scanner designs, and image processing algorithms.  Improved performance in the instrumentation is especially important for heavy patients where image quality suffers due to increased attenuation and scatter effects.  New scanner designs incorporate scintillators with improved energy resolution which helps to reduce scattered radiation, and excellent timing resolution which makes it feasible to incorporate time-of-flight (TOF) information into the image reconstruction algorithm, a technique that improves signal-to-noise.  Results of experimental measurements are combined with clinical patient studies to quantify the improvements made possible with the new technology and to demonstrate the benefit of time-of-flight with respect to lesion detection, lesion uptake, and characterization of disease. Details will be given to explain how the acquisition and reconstruction protocols are optimized with the Philips Gemini TF PET/CT scanner to take advantage of the TOF capability of the PET/CT scanner. A summary will also be given of ongoing research and development of TOF technology which will lead to further improvements in instrumentation, and eventual translation to clinical applications.
