Hi all,
Here is the final plan for the Day of Mourning activities. Please review the agenda below to help guide your participation throughout the day.
Agenda
9:58–10:00 – PA announcement and regroup test
10:00–10:30 – Divisional safety talks
10:30–12:00 – Activity stations (see attached map for locations)
12:00–1:30 – Lunch break
1:30 onwards – All Hands (including moment of silence), guest speakers, divisional talks, CSO address, and Hazard Hunt
What we need from you
* Supervisors: Please inform your teams in advance (activities begin at 10:00 AM)
* Participate actively in discussions on incidents, hazards, and safety concerns
* Engage with all activity booths (Please refer to the attached map for exact locations)
* Attend the All-Hands session (in person or via Teams)
* Promote hazard awareness and reporting
Booth Locations
* Fire Lane: Stretch/mindful movement, OH&S, Radiation Protection, Waste Management, First Aid, Incident Sharing
* Auditorium: Psychological Safety/HR, JHSC, Electrical Safety, Cryogenics
* Other: PPE Vendors (outside Badge Room), Regroup Station (Reception)

Hazard Hunt
* Scan QR code to report hazards around the site via MS Form (photos encouraged)
* Each submission = 1 entry into a lucky draw for TRIUMF merchandise/prizes
* The form will remain open until noon on Monday, April 4
* Lucky Draw Winners will be announced the following week
* All participants with valid submissions will receive a participation prize
* Entries will be reviewed by QESH team to support ongoing safety improvements
* Important Note: Please limit hazard hunt to common areas only (excludes restricted zones such as exclusion areas and work permit-controlled workspaces).
