IMPORTANT REQUEST for Travel Information for approved TRIUMF visits

Dear TRIUMF Users and Visitors,

As TRIUMF progresses through the various stages of its Return to Site plan, please familiarize yourselves with the latest updates to the guidelines which can be found here<https://www.triumf.ca/COVID-19%20RESOURCES>.
The notice below is intended for those visitors who believe they need to come to the TRIUMF site while COViD-19 restrictions are in place. Visitors who do not plan to travel to the lab during this time can safely ignore this message.


TRIUMF has entered the first stages of its Phase 3<https://www.triumf.ca/sites/default/files/TRIUMF-RRU-02062020_21.png> (‘Phase 3a’) Return to Site plan<https://www.triumf.ca/node/35969/>, highlighted by the return of some users from across Canada for essential activities.  While COVID-19 protocols are in place, TRIUMF is restricting on-site occupancy to permit proper physical distancing, and instituting some contact tracing protocols. As such TRIUMF needs to know how many people are on site at any time, where they are active, and how they arrived at the lab.
n.b.   All normal visitor procedures (e.g.  requesting visitor status via the Visitor Management System<https://mis.triumf.ca/admin/visit/index.jsf>) continue to apply.


  *   Prospective visitors are reminded to contact their TRIUMF Contact and to apply for pre-approval for their travel to TRIUMF prior to initiating their travel plans. The applications will be reviewed by the Deputy Director, Research.

     *   The online “Request Pre-approval to Visit TRIUMF” form can be accessed here: https://forms.office.com/Pages/ResponsePage.aspx?id=EDUFwrOceUai049ELgO1h6zvOVrJK0tBmVNqyB5WIcVUMDgzWVU3NU8yUjBFQ05CVjI5TFpOVVlYNy4u

     *   Before you finalize your travel plans, please allow sufficient time (>1 week) for TRIUMF to review and decide on the application. Please note: visitors without pre-approval will not have access to the lab.


  *   Once pre-approved:
     *   Visitors must complete the Return to Site Orientation (TRIDENT login required) found here: https://learning.triumf.ca/#/online-courses/0f29b87b-9b8c-444b-a828-37b0fef64712

     *   Visitors arriving at TRIUMF reception or TRIUMF House (in particular those who travelled by air) are asked to fill out the "Travel Information Form” found here: https://forms.office.com/Pages/ResponsePage.aspx?id=EDUFwrOceUai049ELgO1h6zvOVrJK0tBmVNqyB5WIcVURVNQTVg2N0YxR0JGOUJEQUgwNk9NSFBMSS4u. This form will be cross-referenced against government websites that track COVID cases on airline flights.
