Reminder for the flu clinic today! We’ve gotten word that local graduate students that aren’t enrolled in the TRIUMF healthcare plan are still welcome at the clinic! For local graduate students, fill out all of the flu clinic form except for the insurance info.

We will be hosting this year’s Immunization Clinic on site on Thursday November 4th in the 2nd Floor Auditorium.

Please see the attached supporting information regarding the vaccination to better inform your decision making.

In keeping with guidance from PHSA, this year will require safety protocols to reduce the risk of COVID-19 transmission.  In support of making informed decisions about the vaccination and your health, we’ve provided some information for your review.  On the day of the event, please wear a mask/face covering. There will be physical distancing markers and signage posted to help maintain safe distance while waiting outside the Auditorium. As an additional screening measure, clinic staff will perform temperature checks.

In preparation
1.    Print and complete the attached COVID-19 screening/Flu immunization consent form (double-sided) and bring it with you on day of the event. Employees are required to complete the **Insurance Information** section of the form, which requires:

Sun Life

  *   Group ID: 025205
  *   Member ID: 7-digit personal code
  *   Carrier ID: 16

Pacific Blue Cross

  *   Group ID:  2936
  *   Member ID: 9-digit personal code
  *   Carrier ID: Div 1,  Div 2, or Div 3
Note: Div 1 for retirees under 70, Div 2 for retirees over 70, Div 3 for Grad Students

2.    Identify your time slot for the flu clinic.

TRIUMF Employees & Family (children aged 5 and up), Emeritus, Long-Term Visitors, TRIUMF Innovations Employees, and BWXT Employees:

  *   Last name from A to D – 10:30 am to 11:30 am
  *   Last name from E to J – 11:30 am to 12:30 pm
  *   Last name from K to P – 12:30 am to 1:30 pm
  *   Last name from Q to Z – 1:30 pm to 2:30 pm

For those brining family members, we ask that they be brought to reception to sign into the logbook. As a reminder, all such visitors will need to be accompanied at all times, while on site.

If you feel sick, please do not attend the clinic.

For those TRIUMF employees unable to attend the on-site clinic on November 4th, you can visit the Shoppers Drug Mart at, 5040 University Blvd, Dentistry Bldg. and let them know you’re a TRIUMF employee and they will administer your flu vaccination under your plan.  You should bring proof of employment (SunLife benefits card, or if not applicable a copy of your paystub and photo ID) and coverage is limited to employees only. Please note that this is only while supply lasts.
